MY FORM WONT LOAD FOR ME TO EDIT

I HAVE TRIED looking at it  but i am new to php so i can't figure out what i did wrong  help plz !!!!!!!!!!!!

God, blue large font yelling help with a wall of code not wrapped in code tags = Obnoxious.

 

Where is the class instantiated that relates to $obj? Perhaps nav.php? Can we see it, we can help you troubleshoot from there.
